Affiliate program terms
Effective August 28, 2026. These terms explain how referrals, commissions and responsible promotion work.
1. Program relationship
Approved affiliates may promote EvolveForm using their assigned referral link. Participation does not create employment, agency, franchise or authority to make commitments on EvolveForm’s behalf.
2. Commission
For existing attributed website subscriptions, the standard commission is 30% of each successful monthly payment during a referred customer’s first 12 billing cycles. Commission is earned separately as each payment succeeds. Free app use does not generate commission; in-app Pro purchases are not currently attributed by this website referral system. The rate shown in an affiliate’s dashboard controls if a different written rate is approved.
3. Attribution
Existing website referrals remain subject to the 30-day link window and the attribution terms accepted when they registered. New free app installs and in-app purchases are not currently eligible for website referral attribution. Self-referrals, cookie stuffing and misleading redirects are prohibited.
4. Approval and payouts
Commissions remain pending for 30 days and may be reversed for refunds, disputes, fraud or payment failure. Eligible payouts require completed Stripe verification and a minimum approved balance of $50. Stripe processing and banking timelines may affect delivery.
5. Promotion standards
Affiliates must clearly disclose their financial relationship with EvolveForm, follow platform advertising rules and use truthful descriptions. Affiliates may not promise specific physical results, impersonate a medical professional, send spam, use stolen content or bid on EvolveForm brand terms without written permission.
6. Content and trademarks
Approved brand assets may be used only to promote EvolveForm while the affiliate remains active. EvolveForm names, logos, software and materials remain EvolveForm property and may not be altered in a misleading way.
7. Taxes and records
Affiliates are responsible for their own tax obligations. Stripe may collect identity and tax information or deliver forms where applicable. Affiliates must keep their payout and contact information accurate.
8. Suspension or termination
EvolveForm may reject, suspend or terminate accounts for fraud, abuse, regulatory risk, misleading promotion or violation of these terms. Valid approved commissions earned before termination remain payable unless connected to prohibited activity.
9. Changes and support
Program economics or terms may change prospectively. Material changes will be communicated through the affiliate dashboard or account email. Questions can be sent to evolveformsite@gmail.com.
